Many warnings circulate about cosmetics, clothing and emotional stress, although the scientific evidence is highly uneven. Distinguishing real risks from suspicions allows for better prevention and avoids blaming women who become ill.

Every 19 October, World Breast Cancer Day reminds us of the importance of early detection, research and access to treatments. In Spain, 38,318 new diagnoses in women are estimated for 2026, according to figures released by the Spanish Group of Cancer Patients (GEPAC). The disease remains the most diagnosed tumor among women, although it also has high survival rates when detected and treated in time.

Prevention tends to focus on gynecological check-ups, mammograms and the importance of knowing one's family history. However, it is also worth examining some habits that are part of daily life: having a glass of wine to unwind, smoking occasionally, spending many hours sitting, maintaining a poorly balanced diet or resorting to certain hormonal treatments without periodically reviewing their indication.

Research identifies several modifiable factors related to breast cancer risk. Others, such as age, genetic predisposition and certain reproductive history factors, do not depend on personal decisions. Moreover, around 80% of breast cancers develop in women whose only identifiable risk factor is sex and age, according to the World Health Organization. That is why even a woman who watches her diet, exercises and has never smoked can receive a diagnosis.

Prevention helps reduce probabilities; it never offers an individual guarantee.

Alcohol: one glass also counts

Wine with meals, beer at the end of the day or a glass at dinner are part of widely accepted social habits. Their normalization makes it difficult for many people to associate these drinks with breast cancer risk.

The relationship is backed by scientific evidence. Alcohol increases the risk of developing this tumor and the probability grows with the amount consumed. The effect is also observed with beverages considered healthy due to their association with the Mediterranean diet, such as red wine.

The body transforms alcohol into acetaldehyde, a substance capable of damaging DNA. Mechanisms related to hormonal metabolism are also involved, including estrogens, which play a role in certain breast tumors. Studies from the European project EPIC, coordinated by the International Agency for Research on Cancer, have found an association between alcohol consumption and breast cancer risk across different subtypes.

The practical question is simple: drinking less reduces exposure to a known risk factor. An occasional glass is not the same as regular consumption, but there is also no amount of alcohol that can be recommended as protective against breast cancer.

Tobacco: a risk that goes beyond the lung

The association between tobacco and lung cancer is deeply rooted in public awareness. Its relationship with breast cancer receives far less attention.

Epidemiological evidence also links smoking with an increased risk of this tumor, although the magnitude of the association may vary depending on exposure, age and other factors. The EPIC project has found results consistent with an increased risk in both active smokers and people exposed to environmental smoke. Exposure during early stages of reproductive life, especially between the first menstruation and the first full-term pregnancy, deserves special attention in research on this risk.

Smoking little, doing so only on weekends or living with smokers for years are circumstances that can go unnoticed when assessing cumulative exposure. Quitting smoking remains one of the most important measures for reducing the risk of numerous diseases, including cancer.

Diet: the pattern matters, not an isolated food

The internet abounds with lists of foods that supposedly feed tumors and recommendations to eliminate sugar, soy, dairy or certain oils. Many of these claims oversimplify complex biological processes and lack sufficient evidence to recommend general restrictions.

The evidence points to a more consistent approach: maintaining a healthy weight, consuming a varied diet rich in vegetables, fruits, legumes and whole grains, and limiting high-calorie-density products that promote sustained excess energy intake.

Excess body fat becomes especially relevant after menopause. Adipose tissue contributes to estrogen production and participates in metabolic and inflammatory processes related to the development of certain tumors. That is why postmenopausal obesity is associated with a higher risk of breast cancer.

Physical activity also matters. Regular exercise is linked to a reduced risk, regardless of body weight, and contributes to maintaining metabolic health during aging.

Regarding sugar, there is no solid evidence that consuming a sweet food directly causes breast cancer. Excessive consumption can promote a positive energy balance and weight gain, which does have relevance for cancer risk. Nor is there justification for eliminating specific foods or resorting to restrictive diets with the promise of preventing this tumor.

The recommendation of the European Code Against Cancerr is to prioritize plant-based foods, limit processed meats and reduce highly caloric foods rich in fats or sugars.

Cosmetics, deodorants and bras: what we really know

Daily exposure to chemical substances raises legitimate questions. We use cosmetics, perfumes, hygiene products and plastic materials for decades, and some substances present in these products can interfere with the endocrine system under certain conditions. Research is studying whether certain exposures could contribute to the development of hormone-dependent tumors.

However, finding a substance with hormonal activity in the laboratory does not prove that the usual use of a product containing it causes breast cancer. To establish that link, it is necessary to evaluate the dose, the route of exposure, the duration, the evidence in humans and other possible confounding factors.

Antiperspirants and aluminum. The concern that products applied to the underarms may promote breast cancer has been circulating for years. To date, available studies have not demonstrated a causal relationship between the use of antiperspirants containing aluminum compounds and this tumor. The American Cancer Society and the U.S. National Cancer Institute consider that the evidence does not support that claim.

Underwire bras. Nor is there convincing scientific evidence that underwire bras, tight clothing or prolonged use of certain garments cause breast cancer. Pressure on the breast has not been established as a causal mechanism of this disease.

Cosmetics and endocrine disruptors. Some substances used in consumer products have drawn scientific interest for their ability to alter hormonal signals. It is a field that deserves regulatory oversight and better exposure studies. The available evidence, however, does not allow us to state that the ordinary use of perfumes, creams or makeup is a proven cause of breast cancer. Reducing unnecessary exposures may be a reasonable preference, but it should not be presented as a prevention strategy whose effectiveness is proven.

The distinction is important: investigating a potential risk is necessary; turning a hypothesis into a definitive medical warning can generate fear without providing real protection.

Hormones and menopause: a decision that requires medical context

Hormonal treatments deserve special attention because they intervene in a physiological process fundamental to women's health.

Menopause hormone therapy can relieve symptoms that considerably affect quality of life. Its balance between benefits and risks depends on age, the time elapsed since menopause, the type of treatment, the dose, the duration and personal history.

Combined estrogen and progestogen therapy is associated with an increased risk of breast cancer, especially with prolonged treatments. The risk varies depending on the formulation and cumulative exposure. Estrogen-only treatments present a different profile, which also depends on clinical circumstances and, among other factors, on whether the woman retains her uterus.

Therefore, it is advisable to periodically review the indication, duration and alternatives with a healthcare professional. The decision should not be based exclusively on fear of cancer or on the promise of rejuvenation, but on an individual assessment of benefits, risks and preferences.

The same caution is necessary with hormonal contraceptives and other medications that modify hormonal exposure: their possible relationship with breast risk should be interpreted according to the preparation and each patient's situation.

Can emotional stress or a toxic relationship cause breast cancer?

It is one of the most delicate questions and also one of those that most easily lead to wrong conclusions. Many women diagnosed with breast cancer recall periods of intense suffering: a separation, the loss of a loved one, family problems, work exhaustion or years of emotional tension.

The temporal coincidence invites us to look for an explanation. However, research has not consistently demonstrated that psychological stress, by itself, causes breast cancer.

A systematic review and meta-analysis published in 2026, with prospective studies on stress and the development of various tumors, found no statistically significant association between psychological stress and breast cancer incidence. The authors noted that the quality of the evidence remains limited and that better studies are needed to resolve some uncertainties.

This does not mean that emotional well-being is unimportant. Chronic stress can disrupt sleep, increase alcohol or tobacco consumption, hinder physical activity, and promote less healthy eating habits. It can also complicate access to medical care or cause a person to postpone appointments and tests. These are plausible indirect pathways, but they do not amount to demonstrating that an emotion caused a tumor.

Abusive relationships, constant fear, or lack of support can have serious consequences for physical and mental health. They deserve attention and specific resources, regardless of whether a causal relationship with breast cancer can be established.

It is also advisable to avoid a particularly harmful message: suggesting that a woman became ill because she accumulated resentment, failed to manage her emotions, or lived with a negative attitude. There is no scientific basis for attributing that responsibility to her. Nor is there evidence that maintaining a positive attitude guarantees avoiding cancer or preventing its progression.

Sleep, night shifts, and biological rhythms

Sleeping little and working at night are part of many people's routines. Disruption of circadian rhythms can alter hormonal and metabolic processes, which is why the relationship between night work and cancer has been the subject of research.

Results depend on the type of exposure, duration, and study characteristics. It is incorrect to state that sleeping poorly for a few nights causes breast cancer, or that any night work necessarily implies an elevated risk. Research on circadian disruption and cancer continues, and conclusions must be adjusted to the evidence available for each specific exposure.

Keeping reasonably regular sleep schedules, aiming for sufficient rest, and addressing persistent sleep disorders are sensible recommendations for general health. They should not be presented as a specific guarantee against breast cancer.

Which measures make the most sense

Effective prevention combines healthy habits, individual risk assessment, and early detection. The everyday measures with the strongest support include reducing or avoiding alcohol, not smoking, maintaining regular physical activity, and preventing excess weight, especially after menopause.

It is also important to know your family history and to consult if there are several cases of breast or ovarian cancer in the family, diagnoses at early ages, or a history of certain hereditary mutations. In those circumstances, the medical team can assess genetic counseling and risk-adapted follow-up strategies.

Mammography remains an essential tool for detecting tumors before they produce symptoms. The starting age and frequency of screening should be adjusted to the corresponding screening program and each woman's history. Any new lump, change in breast shape, skin alteration, or abnormal nipple discharge requires medical evaluation, even if the last mammogram was normal.

The goal of reviewing everyday habits is not to live in constant worry about every product or gesture, nor to turn prevention into an endless list of prohibitions. It is to identify exposures that we can actually modify, demand evidence before accepting alarmist claims, and make it easier for women to access reliable medical information.

Research has advanced considerably in the diagnosis and treatment of breast cancer, but it remains impossible to explain every case from a single cause. Responsible prevention begins by recognizing that complexity and focusing efforts on the measures with the best evidence.

Alcohol, tabaco, sedentarismo, exceso de peso y determinados tratamientos hormonales figuran entre los factores que pueden modificar el riesgo.
