Causes, habits and treatment protocols according to each case

Joint pain is often attributed to aging, an old injury, or accumulated wear and tear. That explanation is insufficient to understand two of the diseases that most limit mobility: osteoarthritis and the various forms of arthritis. Age and genetic predisposition matter, although metabolism also plays a role, as do body weight, physical activity, smoking, certain infections, and the immune system's response.

Medicine is advancing on two particularly interesting fronts: identifying people at risk of developing inflammatory arthritis before irreversible damage appears, and finding treatments capable of modifying the progression of osteoarthritis. The most recent results, however, require distinguishing between clinical improvement, prevention of progression, and actual cartilage regeneration.

Two diseases that share symptoms but not mechanisms

Osteoarthritis is a disease of the entire joint. It affects the cartilage, the subchondral bone, the synovial membrane, and the structures that stabilize movement. The process can combine structural deterioration, local inflammation, and changes in pain perception. Knees, hips, hands, and spine are among the most frequently affected areas.

Arthritis, by contrast, refers to a group of diseases that cause joint inflammation. In rheumatoid arthritis, the immune system participates in attacking the synovial membrane; in psoriatic arthritis, inflammatory mechanisms associated with psoriasis are involved; and in reactive arthritis, the disease can appear after an infection. Gout results from the accumulation of urate crystals in the joints.

This difference determines treatment. In rheumatoid arthritis, controlling immune activity early can prevent bone erosions and disability. In osteoarthritis, the approach combines therapeutic exercise, control of risk factors, pain treatment, and, when appropriate, surgery. A single protocol cannot address all these situations.

Genetic predisposition and biological aging cannot be modified at will. Habits, exposure to certain risks, and the timing of seeking medical care offer, by contrast, opportunities for intervention.

Body weight is one of the best-studied modifiable factors in knee and hip osteoarthritis. Mechanical load matters, although adipose tissue also participates in the production of metabolic and inflammatory signals. In people with overweight or obesity, reducing weight can decrease pain and improve function. The recommendations from the UK's NICE indicate that losing 10% of body weight may provide more benefits than losing 5%, when weight loss is indicated.

Sedentary lifestyle poses another problem. Muscles help stabilize the joints and allow loads to be distributed better during movement. Therapeutic exercise, adapted to each patient's condition, is a central intervention in the treatment of osteoarthritis. When starting, it may cause transient discomfort; regularity and progression adjusted to individual capacity are essential. The updated EULAR European recommendations on physical activity for people with osteoarthritis and inflammatory arthritis reinforce this strategy.

Smoking deserves special attention in rheumatoid arthritis. Its relationship with the risk of developing the disease is well documented, especially in people with certain genetic predispositions. Exposure to smoke can promote immune processes related to the production of autoantibodies. Quitting smoking is a reasonable preventive measure, although it does not guarantee that a person will not develop the disease.

Diet also plays a role, mainly through metabolic health, weight control, and the overall quality of the diet. A pattern rich in vegetables, legumes, fruit, nuts, whole grains, and unsaturated fats helps maintain cardiovascular and metabolic health. In gout, moreover, diet and alcohol can influence urate levels, but medical treatment aimed at reducing them is usually decisive.

It is advisable to avoid promises of anti-inflammatory diets capable of reversing any joint disease. The evidence better supports combining healthy habits with specific treatments than the indiscriminate elimination of foods or the use of supplements without a defined clinical indication.

The gut enters joint research

The gut microbiota is another relevant line of research. Studies on rheumatoid arthritis have identified associations between bacterial composition and immune activity. Among the microorganisms investigated are Prevotella copri and Collinsella aerofaciens, while butyrate-producing bacteria, such as Faecalibacterium prausnitzii, are being studied for their possible role in maintaining the intestinal barrier and immune regulation.

The gut-joint axis hypothesis proposes that certain microorganisms and their metabolites can modulate systemic inflammation. In osteoarthritis, the relationship between microbiota, metabolism, and disease progression is also being investigated.

The limitation is important: an association does not demonstrate causality. There is still no commercial microbiota test that can reliably identify which bacterium is causing a person's joint pain, nor a proven probiotic protocol capable of reversing osteoarthritis. Microbiota analyses may be of interest in research, but they should not replace rheumatological evaluation or guide antimicrobial treatments without indication.

Early diagnosis consists of recognizing the correct clinical pattern and requesting the tests that can change medical management.

In osteoarthritis, diagnosis is usually based on symptoms and physical examination. According to NICE, in people aged 45 or older with activity-related pain and no morning stiffness or morning stiffness lasting up to 30 minutes, a clinical diagnosis can be established without X-rays. Imaging is reserved for atypical situations, diagnostic uncertainty, or suspected alternative disease. Symptom intensity and functional limitation matter more than an isolated image.

In inflammatory arthritis, warning signs include persistent swelling, prolonged morning stiffness, pain in multiple joints, loss of function, and sometimes fatigue. Rheumatoid arthritis can begin with relatively mild symptoms before producing structural damage.

The workup may include erythrocyte sedimentation rate, C-reactive protein, rheumatoid factor, and anti-citrullinated peptide antibodies (anti-CCP or ACPA). Musculoskeletal ultrasound can detect synovitis and Doppler signal compatible with active inflammation; magnetic resonance imaging can identify subclinical changes in certain patients. None of these tests should be interpreted in isolation: a negative result does not always exclude inflammatory arthritis, and a positive autoantibody does not necessarily mean the disease will develop.

Risk assessment before clinical rheumatoid arthritis appears is one of the most promising fields in rheumatology. However, indiscriminate screening of autoantibodies in the general asymptomatic population is not recommended. Patient selection and interpretation of results require specialist judgment.

Prevention of rheumatoid arthritis is already being studied in clinical trials

Research published in The Lancet and trial follow-up through 2026

<img decoding="async" src="https://images.openai.com/static-rsc-4/60iBteuNSXlzDfCttMFstzx_kcmD3FPZsIrgVbVHvzXHWdRctZgjUfV1Gj8wxcsw7_njsviqR-WUFqss8egrJUdgwMcpy0jSypyQwoQ7xoQ8_T_00c4f_3d7Te59NwotMQUo3Vmq_IpYPBSPIkd0CgpruH3i1Nx-mjYESyItGgk?purpose=inline" alt="In conversation with... The APIPPRA trial showed a reduction in progression during the treatment period. Long-term follow-up, published in 2026, found that the difference in arthritis-free time remained significant at four years, although the benefit had diminished.

The clinical interest lies in intervening during an identifiable risk phase. Abatacept is an immunomodulatory medication that requires medical selection and safety monitoring; these results do not justify administering it preventively to anyone with joint pain.

Georg Schett and the ARIAA study

Professor Georg Schett, from the University of Erlangen-Nuremberg, participated in the ARIAA trial, published in The Lancet in 2024. The study evaluated abatacept in people with ACPA-positive antibodies, joint pain and signs of subclinical inflammation detected by magnetic resonance imaging. The results indicated a reduction in inflammation and a delay in progression to rheumatoid arthritis in the treated group.

This strategy, known as disease interception, attempts to act before established arthritis causes permanent damage. It remains a specialized area of research and is not equivalent to having a universal preventive treatment.

The 2026 research extends this line with serum proteomics studies. An analysis of samples from the APIPPRA trial identified differences in 80 proteins between people who developed rheumatoid arthritis and those who did not progress, with observable changes in the two years prior to clinical onset. The SomaScan 7k platform was used to measure thousands of proteins. It is a promising finding for developing risk models, not a screening test already validated for general clinical practice.

Osteoarthritis: the great challenge of repairing cartilage

Innovation in osteoarthritis pursues a more ambitious goal than reducing pain: modifying the disease and preserving or restoring joint structure.

A review published in the European Journal of Pharmaceutical Sciences in February 2026 examined the clinical trials of intra-articular drugs aimed at stimulating cartilage matrix formation. It analyzed candidates known as DMOADs (disease-modifying osteoarthritis drugs), including treatments aimed at stimulating anabolic cartilage processes. Its central conclusion is that there is still no approved intra-articular drug that has been shown to reliably restore cartilage matrix in osteoarthritis.

This requires cautious interpretation of commercial regenerative medicine offerings. Platelet-rich plasma, hyaluronic acid injections, stem cells and tissue-derived products are frequently marketed as regenerative solutions, but their efficacy varies depending on the procedure, the indication and the quality of the studies. A reduction in pain does not demonstrate that cartilage has been regenerated.

The same caution applies to experimental therapies that act on cellular senescence, inflammatory pathways or tissue repair mechanisms. They are relevant fields for longevity, although results in animals, cell cultures or early studies cannot be presented as a demonstrated reversal of human osteoarthritis.

Which innovative treatments deserve attention in 2026

The update of the recommendations from the American College of Rheumatology (ACR), published on September 14, 2026, offers another angle of interest: the incorporation of conditional recommendations for GLP-1 receptor agonists in knee osteoarthritis and for methotrexate in erosive hand osteoarthritis. At the same time, the document reinforces exercise, self-care, weight loss when indicated and shared decision-making.

A conditional recommendation does not mean that these drugs should be prescribed to all patients with osteoarthritis. Their usefulness depends on the clinical profile, the authorized indications, the risks and the evidence available for each situation. In particular, GLP-1 medications should not be used exclusively to treat osteoarthritis without an appropriate medical indication.

To evaluate the innovative protocols offered in longevity clinics, it is helpful to distinguish four goals:

Symptomatic control: reducing pain, stiffness and inflammation.

Disease modification: reducing inflammatory activity or slowing structural damage.

Functional recovery: improving strength, mobility, stability and capacity for daily activities.

Structural regeneration: demonstrating that previously damaged joint tissue has been restored.

A protocol can achieve good results in the first three goals without demonstrating the fourth. This distinction is essential for evaluating biological treatments, infiltrations, advanced rehabilitation and regenerative medicine programs.

Can they reverse it?

The answer depends on what we understand by reversal. A person can experience less pain, regain mobility and significantly improve their quality of life, and can also achieve better muscle function and reduce metabolic factors that contribute to joint overload.

However, established structural osteoarthritis currently has no proven treatment that guarantees complete regeneration of lost cartilage. Research into disease-modifying drugs, tissue repair and cell therapies attempts to fill that need, but significant clinical limitations remain.

Inflammatory arthritis offers another perspective. In rheumatoid arthritis, achieving clinical remission through early treatment can halt or considerably reduce the progression of damage. Remission does not necessarily mean that existing erosions disappear, but it can prevent future lesions and preserve function. The 2025 EULAR recommendations, published in 2026, insist on starting disease-modifying drugs as soon as the diagnosis is established.

Therefore, the most solid opportunity for preventive medicine is not to promise that all joints can regenerate, but to identify the responsible mechanisms, intervene before the damage is irreversible and measure results with objective criteria.

A serious joint prevention and monitoring program should begin with a medical history, a physical examination, and an assessment of risk factors. From there, tests and interventions should be selected based on the diagnostic suspicion.

Area

Assessment or intervention

Diagnosis

Pain pattern, stiffness, swelling, history, examination, and targeted tests.

Metabolic risk

Weight, blood pressure, glucose or HbA1c and lipid profile, as applicable.

Inflammation

CRP, ESR and autoantibodies when inflammatory disease is suspected.

Imaging

X-ray, ultrasound or MRI depending on the clinical question, not as an indiscriminate panel.

Treatment

Therapeutic exercise, rehabilitation, metabolic control and specific medication for the diagnosed disease.

Follow-up

Pain, function, mobility, inflammatory activity, adverse effects and, when indicated, structural progression.

The microbiota, metabolomics and proteomics could provide additional information in selected patients or within research projects. However, these technologies do not yet replace validated clinical tools for diagnosing and monitoring osteoarthritis and arthritis.

The most important measure for the patient remains recognizing the signs that require medical attention. A joint that suddenly becomes hot, red and swollen, especially if accompanied by fever, needs urgent assessment to rule out joint infection. Persistent swelling or prolonged morning stiffness also warrant an early rheumatology evaluation.

El dolor articular suele atribuirse al envejecimiento, a una lesión antigua o al desgaste acumulado.
